Looking for Medicare doctors and nurse practitioners in Colorado Springs? Or primary care tailored for patients 60 years and older? You’ve come to the right place and welcome to AgeWell Medical Associates, PC.
Under the visionary leadership and direction of Jeffrey Kulp, MD, AgeWell Medical Associates offers innovative patient-centered care to meet the increasing demand for primary care services in the Colorado Springs area. Our model for primary care for older adults is built on a team-based approach.
Our Nurse Practitioners are primary care providers and see their own patients. They can diagnose, develop a treatment plan, prescribe medications, order tests, and refer to specialists when needed. Our providers work collaboratively with Dr. Kulp and consult him as needed.

AgeWell is currently recognized by the National Committee for Quality Assurance (NCQA) as a Patient Centered Medical Home (PCMH). We want to be a center of excellence for the older adult.
The PCMH concept equips primary care practices to do their part in achieving the ‘quadruple aim’ of health care reform – better patient care and experience, population health, reduced costs and provider satisfaction.
