An AgeWell provider is on call 24 hours and 7 days a week, even after office hours. If you have an urgent problem and need to speak to a provider after business hours, call the office phone number and follow the prompts to page the on-call provider.

Primary CareEmergency Care Follow Up
Emergency rooms are best utilized for life-threatening emergencies. Some examples of this are chest pain / heart attack, stroke-like symptoms, uncontrolled bleeding, respiratory distress, and unconsciousness.
Our commitment to our patients is that AgeWell is available first. A call to us can avoid a lengthy and unpleasant (not to mention costly) experience in the Emergency Room. If you’re uncertain whether you should visit the Emergency Room or not, call our office first.
We also have a provider available after business hours and on weekends who will properly advise you as to the best course of action.
If you are seen in the ER or urgent care, please call AgeWell to notify us and schedule a follow up appointment.
Hospital / Facility Care Follow Up
AgeWell has dedicated staff to follow patients when they are admitted to our local hospitals. Our providers no longer see patients in hospitals or rehabilitation facilities, so we may not be aware when you are admitted.
If you are hospitalized or admitted to another facility, we ask that you call us as soon as you are discharged. This allows us to promptly schedule appropriate follow up care for you, within 14 days of discharge.
