I’m Jeff Kulp.
I’d like to tell you about a new benefit that Medicare and Medicare Advantage are offering you.
It’s called advanced primary care management.
Primary care is what we do here at AgeWell Medical Associates.
We are your point of first contact when you have a health issue.
We help you manage all your health conditions and stay healthy with preventive care.
We do as much of your healthcare as we are able to.
When we need to involve a specialist we coordinate that care for you.
We have a plan for you to maintain your health which we update every year at your annual wellness visit.
What then is advanced primary care?
Consider that your primary care provider is working for you, caring for you, even when you’re not in the office.
We’re available by phone, video, and through the portal.
Your provider is always monitoring the care you are getting from specialty providers and in other care settings, the emergency department, urgent care, hospitals, rehabilitation facilities.
That way your provider is always up to date with all the care you are getting, all the testing you are having done, and any changes that may need to be made in your medication.
So your primary provider can truly be your partner in managing your health.
When we’re not in the office, we’re available 24/7 by phone.
If you’ve ever tried to reach us outside of office hours, you know how important that service is.
So Medicare has identified these services as advanced primary care management.
It’s all the services we already provide to you at AgeWell, the kind of care you have come to expect from your AgeWell provider.
So if you’re already receiving these services from your AgeWell provider, why do you need to enroll in advanced primary care management?
Medicare requires that you and your provider have a written understanding that you want and are getting this advanced care.
Once that agreement is in place, Medicare will pay your provider a monthly fee.
Why does your provider need a monthly fee to care for you?
It’s well known that independent primary care practices, not owned by hospitals or private equity or multi-specialty groups, practices like AgeWell are in financial peril.
It costs more money to provide these comprehensive services than can be generated by fees for office visits alone.
By your enrollment in advanced primary care management, your AgeWell provider will receive additional income that is essential for AgeWell’s continued operation.
Will there be additional costs to you?
Depending on your insurance there may be a monthly co-payment.
Your provider and our billing staff can answer your questions about that.
